On behalf of the Australian Fragility Fracture Alliance and Osteoporosis NZ, the SOS Fracture Alliance is please to share the below update from the ANZ Fragility Fracture Registry about the release today of their 2nd Annual Report.
~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~
I am delighted to share the second annual report of the Australian & New Zealand Fragility Fracture Registry (ANZFFR). This report contains data for over 15,000 people recruited to the Registry in New Zealand between 1st July 2023 and 30th June 2024 with 16-week follow up of participants to 31st October 2024. Also, for the first time, we are reporting on 52 week follow ups of people recruited the year before.
In New Zealand, Fracture Liaison Services (FLS) have had publicly funded support from the Accident Compensation Corporation (ACC) since before the launch of the ANZFFR in March 2022 which has helped us achieve near-complete national FLS coverage and Registry participation.
The situation is quite different in Australia where, without defined funding or national Clinical Standards as a benchmark for care quality, progress has been much slower.
The ultimate goal of the ANZFFR is to use data to improve health system performance and optimise outcomes for people with fragility fractures by improving secondary fracture prevention, reducing rates of further fragility fractures and their associated morbidity and mortality. This will be achieved by:
Evaluating Fracture Liaison Services performance against the Clinical Standards for
Fracture Liaison Services in New Zealand, published in December 2021, www.osteoporosis.org.nz.
Preventing future fragility fractures by monitoring secondary prevention interventions.
Standardising care across Australia and New Zealand by addressing barriers to the use of the best available evidence.
Providing publicly available information so that patients can be reassured they receive the standard of care they need after a fragility fracture.
Providing data for research questions or projects, nationally and internationally, as required.
This year, whilst still early, we can start to use this data to answer the important question –
are we doing something useful here, are we making a difference?
In New Zealand, the Registry has built strongly on a very successful first year, raising our “capture rate” of predicted fragility fractures each year from 55% in Year 1 to 72% this year.
Key highlights from the report include:
>90% of patients having a bone health and falls risk assessment.
>80% treatment recommendation rate within 12 weeks post-fragility fracture.
>80% of follow ups conducted within 16 weeks.
At 52 week follow up, refracture rates (i.e. further fractures in people participating in the Registry after fracture prevention treatment recommendation) are substantially – but not yet statistically significantly – lower than would be predicted from comparable international data.
As with all quality improvement (QI) programmes, there is room for improvement. There are large variations in practice between FLS teams across NZ, but this is both predictable and exactly the scenario that QI projects are designed to address. Every FLS has its strengths and weaknesses, and all can learn from each other. With FLS coverage now reaching 98% of the NZ population, we have a unique opportunity to bring fracture care and prevention for every New Zealander up to best practice standard.
The ANZFFR would like to thank all the staff in the 20 New Zealand and two Australian Fracture Liaison Service teams, ACC’s Injury Prevention team and all those working to provide good quality care to people with fragility fractures for their dedication and hard work.
We hope that their passion for their patients and commitment to quality improvement is
visible in this report.
